Emergency ID Tag

In keeping with the photography club’s safety goals, the club has developed an ID Tag program to assist in ensuring that club members receive prompt medical attention in the event of an emergency while on a club sanctioned field trip.  The tags could also be beneficial when out on a self-guided field trip.  Participation is voluntary; however, you are strongly encouraged to take part in this important program.
 
Each member will be given a bright orange ID tag.  Complete the from with your name, medical information, name of primary physician and name of a relative or friend to contact.  The completed form will provide a list of any allergies you have and medications you may be taking, all of which will help and EMT or physician with treatment.  This information could prove invaluable should you require emergency medical attention.

To maintain privacy, fold the orange personal information sheet in half, length wise then fold it in half again.  Insert the form in the plastic sleeve provided and use the loops to secure it to your camera bag.  You may want to include a photocopy of your insurance card along with the ID tag.  Members are urged to keep this with them while on a field trip.

FIRST AID

For general cuts, burses, stings or burns the field trip director has a first aid kit available.   For minor pain or headaches, Advil, Excedrin and Aspirin are available.
